Upon admittance, all the following forms must be completed in their entirety and submitted by May 15th, (late processing fee of US$50 if deadlines have passed).

NOTE: PER PUBLIC HEALTH LAW, NO INSTITUTION (CAMP) SHOULD PERMIT ANY STUDENT / CAMPER TO ATTEND THE INSTITUTION WITHOUT COMPLYING WITH THIS LAW.
